How long do ED medications last? The practical answer depends on the drug, dose, meal timing, metabolism, and what “lasting” means. Sildenafil and vardenafil generally provide a shorter treatment window measured in hours, while tadalafil can support responsiveness for substantially longer. None produces an automatic or continuous erection. Phosphodiesterase type 5 (PDE5) inhibitors improve the normal erectile response to sexual stimulation, and their effect gradually diminishes as the body clears the medicine.

How Long Do ED Medications Last in the Body?

Three time measures are often confused: onset, treatment window, and elimination half-life. Onset is the earliest point at which a medicine may begin supporting an erection. The treatment window is the interval during which clinical studies have observed improved erectile responsiveness. Half-life is the time required for the blood concentration to fall by half. These measures overlap, but they are not interchangeable.

For standard oral formulations, the approximate profiles are:

The 2025 European Association of Urology guideline update continues to recommend PDE5 inhibitors as first-line drug therapy for erectile dysfunction and emphasizes choosing treatment according to clinical factors and patient priorities.[1] A 2026 network meta-analysis of 83 randomized trials likewise found that approved on-demand PDE5 inhibitors improve erectile function, while confirming meaningful differences in pharmacokinetics and adverse-event profiles.[2]

A treatment window also should not be interpreted as the duration of an erection. An erection lasting more than four hours is a medical emergency called priapism and requires immediate care. The intended effect is a temporary increase in the probability of achieving and maintaining an erection when sexual stimulation occurs.

Sildenafil: Shorter Window and Meal-Sensitive Absorption

Sildenafil is usually taken roughly one hour before anticipated sexual activity. Some men respond within 30 minutes, while others need longer. Its plasma half-life is approximately four hours, which aligns with the commonly cited four- to six-hour practical window. Clinical responsiveness can sometimes be detected as late as 12 hours after a dose, but the effect is generally strongest earlier and becomes less predictable as concentrations decline.[1]

Food is one of the most important variables. A high-fat meal can delay the time to peak concentration by about an hour and reduce the peak blood concentration. That does not necessarily make the medicine ineffective, but it can make onset slower or the response seem weaker. Taking sildenafil after a heavy dinner and judging it 20 minutes later is therefore a poor test of whether the medication works.

Alcohol can add another source of variability. Modest alcohol consumption does not create the same pharmacokinetic interaction as a high-fat meal, but larger amounts can impair erection physiology independently, reduce arousal, and increase dizziness or low-blood-pressure symptoms. Men should follow their prescriber's instructions rather than repeatedly increasing a dose in response to a meal or alcohol-related delay.

Sildenafil's shorter profile may suit men who prefer a defined treatment period. It also requires more deliberate timing than tadalafil. A 2026 systematic review concluded that approved PDE5 inhibitors are broadly effective, with the choice among them appropriately individualized according to onset, duration, contraindications, and tolerability.[3]

Tadalafil: A Longer Window, Not a 36-Hour Erection

Tadalafil differs most clearly in its long half-life: approximately 17.5 hours. With on-demand dosing, improved erectile responsiveness may begin around 30 minutes after administration, often peaks closer to two hours, and can remain clinically meaningful for up to 36 hours.[1] This longer interval reduces the need to coordinate a dose with a precise moment.

The phrase “up to 36 hours” is frequently misunderstood. It means that sexual stimulation may produce a better erectile response at different points within that period. It does not mean maximum drug effect is constant for 36 hours, and it does not mean an erection persists throughout the window. Concentration declines continuously after the peak, so individual response at hour 30 may differ from response at hour three.

Unlike sildenafil and vardenafil, tadalafil absorption is not meaningfully altered by food. It is therefore more predictable around meals. Its longer residence in the body can also extend adverse effects such as headache, flushing, indigestion, nasal congestion, back discomfort, or muscle aches. Men who experience side effects should contact the prescribing clinician rather than redose or add another ED medicine.

Tadalafil can also be prescribed as a lower once-daily dose. Daily therapy is conceptually different from taking a higher dose as needed: it establishes relatively steady exposure and separates medication timing from sexual activity. Whether daily or on-demand treatment is appropriate depends on frequency of sexual activity, other conditions, concurrent medicines, response, and clinician judgment. The two schedules should not be combined unless specifically prescribed.

Vardenafil: A Short-Acting Alternative With Specific Precautions

Vardenafil has a timing profile broadly similar to sildenafil. Standard tablets are generally taken 30–60 minutes before sexual activity, reach peak concentrations in roughly an hour for many users, and have a half-life around four to five hours. The clinically useful window is usually several hours rather than a full day.[2]

A high-fat meal may delay or reduce absorption of standard vardenafil tablets, although the magnitude can depend on formulation and meal composition. Sexual stimulation remains necessary, and one unsuccessful attempt is not sufficient to establish treatment failure. Incorrect timing, inadequate stimulation, anxiety, heavy food intake, excessive alcohol, and an untreated underlying vascular or hormonal condition can all influence the result.

Vardenafil requires particular attention to cardiac electrical risk. It is generally avoided in men with congenital long-QT syndrome and in those using certain antiarrhythmic medicines that prolong the QT interval. As with sildenafil and tadalafil, it must not be used with nitrates because the combination can cause a dangerous fall in blood pressure. A clinician or pharmacist should review the full medication list, including recreational nitrites sometimes called “poppers.”

The 2026 comparative network meta-analysis found statistically significant efficacy for common doses of sildenafil, tadalafil, and vardenafil versus placebo. It did not establish one universally best medication; dose, tolerability, duration preference, and health context remain central to selection.[2]

Why the Same Medication Can Last Longer or Shorter

Population averages cannot fully predict an individual's response. Several factors alter onset, intensity, or duration:

  • Meal composition: Fat-heavy meals can slow sildenafil and vardenafil absorption.
  • Dose and formulation: Higher exposure may increase both efficacy and adverse effects, but dose changes require prescriber approval.
  • Age and organ function: Liver or kidney impairment can reduce clearance, increasing exposure and sometimes requiring a lower dose.
  • Drug interactions: Strong CYP3A4 inhibitors—including some antifungals, antibiotics, and HIV medicines—can raise PDE5 inhibitor concentrations. Enzyme inducers can lower them.
  • Underlying ED mechanism: Severe vascular disease, diabetes-related nerve injury, pelvic surgery, low testosterone, or substantial performance anxiety may change apparent response.
  • Technique and expectations: Sexual stimulation is required. Taking another dose too early because an erection did not occur automatically increases risk without correcting the misunderstanding.

No PDE5 inhibitor should ordinarily be taken more often than prescribed. Men should not combine sildenafil, tadalafil, vardenafil, or other ED treatments on their own. Although clinicians sometimes use individualized regimens for difficult-to-treat ED, combination exposure can increase headache, flushing, dizziness, low blood pressure, visual symptoms, and priapism. Evidence supporting specialized combination approaches does not make unsupervised mixing safe.

Medical review is especially important for men with unstable cardiovascular disease, recent heart attack or stroke, very low blood pressure, significant liver or kidney impairment, inherited retinal disorders, or complex medication regimens. Chest pain after taking a PDE5 inhibitor requires urgent evaluation; emergency clinicians must be told when the last dose was taken so nitrates are not given inadvertently.

Sildenafil and vardenafil are generally shorter-acting options, with onset commonly within 30–60 minutes and a practical window of several hours. Tadalafil may begin working in a similar timeframe but has a much longer half-life and can support responsiveness for up to 36 hours. Food, alcohol, other medicines, health conditions, and expectations can make the real-world experience differ from these averages.

Duration is only one part of treatment selection. Efficacy, side effects, spontaneity, meal patterns, cardiovascular safety, and the cause of ED all matter.
